Piranhado	When an enemy is afflicted by both a Hex and an Aura Effect, a Piranhado is summoned pulling enemies towards it and dealing X Physical damage over 12 seconds. May occur once every 20 seconds. At Rank 5: Piranhado moves itself towards nearby enemies.

To obtain these elusive powers, players must locate Forgotten Altars scattered throughout Sanctuary. These altars spawn randomly within various dungeons, including:

Nightmare Dungeons: High-tier dungeons offering increased challenges and rewards.
Roothold Dungeons: Newly introduced in the Season of Witchcraft.
The Pit: A time-constrained endgame dungeon that unlocks different levels of Torment difficulty.

The spawn rates for Forgotten Altars vary based on the dungeon's Torment level:
